General Office Policies, Information, and Insurance

Making positive changes to your lifestyle is the cornerstone of good health. These policies are put into  place to help you understand my services and be successful on your journey. I look forward to helping you  reach your health goals by creating a healthy eating and lifestyle plan! Sessions are conducted virtually or by phone.

An Apple A Day Nutrition Needs LLC Cancellations, Appointments, Program Policies, and Insurance:

  • Kindly cancel appointments at least 24 hours in advance of the scheduled appointment time. You will be charged a $50.00 cancellation fee if less than 24 hours notice is given.

  • If you do not show for your scheduled appointment you will be charged a no-show fee for the full amount of the missed appointment.

  • Please be on time for your appointment. If you are late to your scheduled appointment that missed time cannot be made up and you are responsible for the full appointment fee.

  • For your safety as well as others, no driving and/or operating a motorized vehicle during sessions. 

  • I accept Visa, MasterCard, Discover, HSA and FSA cards, and debit cards.

  • A credit/debit card on file is required to secure an appointment. 

  • I do not offer refunds. 

  • I can provide a Superbill for you to submit to your insurance for possible reimbursement.  

  • I am an in-network provider with Regence Blue Cross Blue Shield insurance and provide preventive care for lowering cholesterol and/or reducing blood pressure only. Please note it is your responsibility to call Regence BCBS prior to your first appointment with me to verify your insurance coverage.  When you call the insurance company, please note the date and name of the representative.  Please make sure you have telehealth/video benefits, and give the representative the nutrition service CPT codes: 97802 (initial visit) and 97803 (follow-up visit).  If they need a diagnosis code, please give them the ICD-10 code: z71.3 which is a preventive code to help to maximize your number of allowed visits. Be sure to ask: Do I have telehealth/video visit coverage? Number of dietitian visits allowed? Do I need a referral? Is there a co-pay? Is there a deductible?  An Apple A Day Nutrition Needs LLC
